Complex Diabetes Management
We provide specialised Diabetes Management services for participants who require support to manage their condition safely and effectively. Our experienced team ensures that participants with diabetes receive the care they need to maintain their health, prevent complications, and lead fulfilling lives.
What is Complex Diabetes Management?
Diabetes management involves monitoring and controlling blood sugar levels, administering medications, and making lifestyle adjustments to prevent complications such as hypoglycaemia, hyperglycaemia, and long-term damage to organs. Effective management is essential for participants with diabetes to maintain stable health and prevent serious conditions such as cardiovascular issues, neuropathy, or kidney disease.
Our Approach
We take a person-centred approach to Diabetes Management, ensuring that every participant’s needs are met with dignity and respect.
Our care plans are created and monitored by a Registered Nurse, ensuring individualised care that addresses each participant’s specific diabetes management needs. These care plans are regularly reviewed and updated, with progress notes prepared every three months and care plan reviews every six months.
We work closely with participants, their families, and healthcare providers to ensure all aspects of diabetes care are coordinated and effectively managed.
NDIS Support Category and Code
Our Diabetes Management services are part of the NDIS Core Supports category, specifically under High Intensity Daily Personal Activities (NDIS Support Code: 0104). This category covers the support required for participants who need specialised, high-intensity care, such as diabetes management.
Pricing
Our services are aligned with the NDIS Pricing Arrangements and Price Limits for High Intensity Daily Personal Activities (Code 0104).
